Step 2:

A member of our Shared Lives team will contact you and once an initial discussion with our service has taken place, a member of the Shared Lives team will post an information pack to you providing you with information and advice about the Shared Lives scheme.

Step 3: Pre-Assessment visit

Once you have received the information pack and you decide to continue with your Shared Lives carer application you will required to complete the on-line application form .

Following this visit if you decide not to proceed with your application, no further action will be required.

Step 4: Application

Once we have received your completed application we will contact you to arrange a meeting at a mutually convenient time and place.

Step 5: Health and Safety check

A health and safety check will be completed as a home visit and this is the start of the assessment process.

Step 6: References and DBS check

References will be obtained from the information you provide on your application form. A member of the Shared Lives team will arrange to collect necessary documentation in order to complete the Disclosure and Barring Services (DBS) check.

Step 7: Training

Training starts during the assessment process and will be ongoing in different formats following approval.

Step 8: Panel Review

Once all necessary checks and references have been received by the Shared Lives team a full assessment report will be produced which will be presented to the Shared Lives Independent Panel members for them to consider your suitability to be a Shared Lives carer. If you have progressed this far through the process, we are confident most people, at this stage, will be approved. You will be contacted by the service about the decision made by the panel.

Step 9: You are now a Shared Lives Carer

Once the Panel has approved your application, you are now a Shared Lives carer. At this stage, we will begin the matching process in order to make successful arrangements.

Step 10: Ongoing support

The support does not stop here. Now that you are a Shared Lives carer you will enjoy regular support and encouragement from a member of the Shared Lives team throughout your career as a Shared Lives carer, including supervision and peer support groups.
