This team works closely with individuals, their families, and carers across disabilities, older people, and mental health services. Our team focuses on strengths-based, outcome-oriented support, helping individuals live independently and safely while achieving their personal goals.
This includes assessing their needs, developing personalised care plans, and coordinating with other professionals and services.
Our adult social workers play a crucial role in helping individuals lead happy and healthy lives and may involve advocating for their rights, providing emotional support, and helping them navigate complex social systems.
Be part of our Community Mental Health Teams, working alongside health professionals to support individuals experiencing mental health issues. AMHPs undergo regular re-accreditation to meet the Council’s statutory requirements for Mental Health Act Assessments.

Domiciliary carers provide essential care within the community, ensuring individuals’ needs are met with dignity and respect by helping with daily tasks such as meal preparation, personal care, and light household duties.
We are proud to provide quality reablement care in our short stay resource centres, The Gouldings and The Adelaide.
Both centres offer a dedicated and skilled staff team who work to support and empower individuals to promote their independence and confidence in a relaxed, social environment.
Join our specialist safeguarding team dedicated to protecting vulnerable adults from harm, abuse, or neglect. Collaborate with internal and external partners to address and manage adult safeguarding concerns, ensuring individuals' safety and well-being.
The transitions service helps young adults move into adult social care from children’s services to get the best support as adults. We work with young people to understand what care and support needs they might have in preparation for adulthood.
The Team works closely with children’s services to ensure that any ongoing support plan is tailored to the individual’s needs.
