This form is to enable parents/guardians apply for their child who is attending an Island school to be assessed for free home to school transport eligibility.

The data collected in this form will be sent to the following: School Admissions and will be shared with: School Transport Team, School Transport Operators i.e. bus companies, taxi companies, School which the child named on the application form attends, Education and Inclusion Service and Isle of Wight Council Financial Services

We will keep your personal data in accordance with our Retention Policy which is available on our website.
