We have exciting opportunities for 2 Social Workers to join our Transitions team that forms part of the Integrated Learning Disability team in Adult Social Care on our stunning Isle of Wight.

- 1x 37 hour (full time) Social Work position

- 1x 23 hour (part time) Social Work position

At #TeamIWC our Transitions team support young people with additional needs with their transition to adulthood. This is an immensely rewarding role that really offers you the professional opportunity to shape the future lives of our Island’s young people and their families for the better.

You will play a pivotal role in supporting young people and importance of the work we do to help them transition into adulthood cannot be underestimated.

Approaching adulthood can be a challenging time for all young people, this is especially true when they have additional needs, there can be lots of factors to consider and plan for.

This can be further impacted on by long-term health conditions, learning disabilities and other needs that mean that an individual would benefit from support to plan their transition from childhood to adulthood to help them recognise and reach their potential.

Within the Transitions team you will work with individuals, their families and their professional networks from a variety services to support a positive transition to adulthood, identifying the strengths the individual has and using these as a basis for an individual to have choice and control over their future, supporting access to services that promote well-being and ongoing development.

Day to day duties include:

- Direct work with young people and their families.

- Completing assessments and reviews to identify eligibility for support under Care Act 2014.

- Working creatively with people with lived experience, their support networks and their community to promote outcomes.

- Engaging in multi-agency working across council and external departments.

- Supporting the wider team to complete statutory duties.

The ASYE is intended to be the first stage of the continuing professional development for Registered Social Workers, and the framework will be closely allied to the Professional Capabilities Framework (PCF).  You will be therefore, be required to satisfactorily complete the ASYE programme within a 12-month period (or 18 months if part time).
