These are exciting time on the Isle of Wight within Adult Social Care, the whole service has supported a redesign that enhances strength-based practice, working with the community and our third sector partners.
We are obsessed with how to best use the strengths of those we serve in designing their care and support – supporting people to regain and maintain the maximum level of independence possible. We are very fortunate to have voluntary and community sectors who we regard as equal partners in the design and delivery of care and support for the people we serve (so that all of our contracts with them are now a for a minimum three years). Which helps support the individual’s recovery and maintain their wellbeing.
Of course, integration with health is a key priority – and being surrounded by water means that we really have to invest in making relationships and care pathways work effectively – as our residents cannot easily, or cheaply, go elsewhere. We have recently published our single Health and Care Plan for the IoW. Equally, we are integrating care management with health across mental health and learning disabilities – and the plan is also to fully integrate commissioning budgets in these areas. So, you will see the evidence and strong ethos of working all partner agencies to produce better and effective resources for people that may use our services.
The Practice Process and Systems Lead is an exciting opportunity to join our practice development team taking the leading on our systems and policy developments whilst managing a busy team and overseeing the transformation of our admin functions.
