Apply for Short Breaks membership
You can apply for a Short Breaks membership card online or by post. It's free to join!
For both types of application, you will need to:
- complete the application form
- provide a passport-sized photograph with your child's or young person’s name and online form reference number written on the back of the photo
- send the photograph to us within 14 days of submitting your form to our freepost address.
Over 700 children and young people now have Short Breaks membership cards.
If you apply by post, download a copy of our Short Breaks application form (PDF, 183.7KB, 4 pages), follow the instructions provided and post to:

Short Breaks membership card
Eligible children will need to register for a Short Breaks
membership card to access activities funded by the
Isle of Wight Council’s Short Breaks programme.
Each Short Breaks membership card contains a unique
reference number which will be specific
to each child or young person.
You will need to show activity providers
your child’s Short Breaks membership
card when booking/attending.
Parents/carers and siblings do not
require a Short Breaks membership
card to support their child or young
person in accessing the Short Breaks programme.
